Naval Branch Health Clinic Dahlgren

Welcome to Naval Branch Health Clinic, Dahlgren, Va. Our mission is to provide force health protection, operational readiness, outstanding customer service and quality health care. The clinic goal is to provide exemplary service to enrolled active duty and family member TRICARE Prime beneficiaries, retired TRICARE Prime beneficiaries, and civil service employees of Naval Support Facility Dahlgren. Services provided include routine and acute health care with limited ancillary support (pharmacy, laboratory, radiology). Dental care is provided to all active duty and includes routine

dental exams and cleanings.

The clinic is closed on national

holidays.

Appointments:

All appointments are scheduled on a same day basis (OPAC – Open Access Appt). The clinic opens as 0730 and we start making appointments then. Appointments are on a first-come-first serve basis. If the schedule is full and a patient is not acutely ill he/she may be asked to call another day. Every effort will be made to see patients on the day they call in. The only exception is Wellness Appointments (physical, Pap, Well Baby check, school/sports PE) they are scheduled in advance. The schedule is opened weekly every Friday. Anyone needing a wellness appt should call on Friday or the following Monday of the week they want to come in. The appointment line is 540-653-8241 or 8242. Patients may also book appointments via the Tricare On Line at www.tricareonline.com.

For Active Duty dental appointments, call (540) 653-8564.

For newborns, call the clinic at (540) 653-8241 as soon as baby is discharged to schedule a newborn follow-up. Be sure to enroll the child in DEERS first.

For emergent care, dial 911 or go to the nearest Emergency Room (ER). There is no prior authorization needed for ER visits. Notify your Primary Care Manager within 24 hours of the ER visit. Call the clinic to schedule any follow-up appointments.

For urgent care, Patients may only go to a Tricare Network urgent care center if the clinic is closed. You must call the next business day to get a backdated authorization.

Potomac Inn - Bachelor Enlisted Housing

The Potomac Inn at 17410 Potomac Inn Road is a military billeting facility, with accommodations for 246 permanent party bachelor enlisted residents. The front desk is located in Building 962; the desk is manned 24 hours a day, 7 days a week. All military personnel must check in at the front desk prior to being assigned a room.

Each occupant in Building 962 resides in a private room equipped with a twin size bed, desk/desk chair, dresser, nightstand and walk-in closet; residents of Building 959 share an open area with twin beds, desk/desk chair, dresser and night stand. All residents share a bathroom and kitchen area, equipped with stove, microwave, refrigerator, washer/dryer, iron/ironing board, vacuum cleaner, and cleaning kit.

Videos and assorted games are available in the entrance lobby for our residents at no additional cost.

Navy Gateway Inns & Suites, Dahlgren

Navy Gateway Inns & Suites Dahlgren is military billeting comprised of four lodging facilities with accommodations for 94 transient officers, enlisted and authorized civilian guests. Also available is a distinguished visiting suite in Bldg. 909.

Our front desk is located at Bldg. 960. It operates 24 hours a day, 7 days a week.

Amenities include wireless high speed internet access, cable TV including HBO, flat screen TVs, DVD player, cordless phone, clock radio, coffee maker, hair dryer, fan, iron/ironing board, microwave and refrigerator. Some suites are equipped with small kitchenettes for extended stays. Laundry facilities are in all buildings.

Customer Service Desk (CSD), Dahlgren

Customer Service Desk (CSD) Dahlgren provides administrative, pay, and personnel support to military tenant commands, including issuance of military, civilian and contractor CAC cards and retirees/dependent ID cards. The Commercial Travel Officer (CTO) is

collocated with CSD and supports all

official transportation and official passport services for the base's military and civilian personnel.

All military pay and personnel services must be coordinated with your local command CPC (Command Pass Coordinator). The CPC will then communicate with CSD via the Transaction Online Processing System (TOPS) to research / answer / process all pay and personnel transaction / questions. No members will come to CSD without talking to your command CPC first unless specifically requested by CSD personnel.

CSD is located three blocks past the main gate on Dahlgren Road in Building 230. Hours of operation are 7:30 a.m. -

3 p.m., Mondays, Tuesdays, Thursdays, and Fridays, and 7:30 a.m. - 1 p.m. on Wednesdays. The telephone number for CSD is (540) 653-8689/7371/7372, DSN 249.

For new arrivals (staff and transients on either PCS or TDY orders), the paragraph above in regards to pay and personnel transactions applies. To complete your check-in process, report to your command CPC immediately upon arrival with your transfer package including all related documents, travel receipts, vouchers and airline/bus tickets.

CSD also houses Common Access Card / DEERS data operations. To make an appointment to get a Common Access Card, visit https://es.cac.navy.mil and click on CSD Dahlgren. Appointments are available daily from 7:30 a.m. - 3:30 p.m. for civilian / contractor card issuance. Active military, retirees and dependents are walk-ins. Military Family Housing

The Navy and Lincoln Military Housing, as a Public Private Venture (PPV) partnership, provide housing for the military in the Dahlgren area. There are 78 units of single-family houses and 125 townhouse units that currently serve the needs of the military families stationed here.

Applications

Applications for housing should be made on an Application for Assignment to Housing (DD Form 1746) with a copy of orders and a copy of current record of emergency data attached. Applications may be submitted by mail to Housing Welcome Center, Naval Support Activity South Potomac (Code N93, Bldg 142), 6136 Jenkins Road, Suite 205, Dahlgren, VA 22448-5109; by fax to (540) 653-7040; or in person at the Housing Welcome Center at any time following receipt of orders to a duty station serviced by Dahlgren.

Housing Referrals

The Housing Welcome Center provides a housing referral service, and incoming personnel are referred to Lincoln Military Housing as a “preferred referral” for the housing located at the activity. Rental and sale property listings are available for those who desire to locate in the surrounding areas.

Temporary Lodging

Because of the limited number and expense of the area hotels/motels,

personnel are encouraged to make

reservations on a temporary basis at the Gateway Inns and Suites at (540) 653-7671 or DSN 249-7671. For information on housing availability, please contact the Housing Welcome Center at (540) 653-8505/8506 or DSN 249-8505/8506.

Public Private Venture

Families desiring to be housed on base will be required to sign a lease with Lincoln Military Housing (PPV partner). This is a six-month lease, and renewed on a month-to-month basis after the initial lease period. Lincoln provides 24-hour maintenance, yard and lawn care, monthly community activities, and a professional staff to serve your needs while living at Dahlgren. They can be reached at (540) 663-2190.

Pets

Keeping cats and dogs is a conditional privilege extended to residents. Residents must comply with state and local laws governing pet ownership, including laws regarding licensing and vaccinations. The limit is two (2) dogs or cats per household. Residents may not keep the following dog breeds in or on the premises: Chows, Doberman Pinschers, Presa Canarios, Pit Bulls (American Pit Bull Terrier, American Staffordshire Terrier, Staffordshire Bull Terrier), Rottweilers, any wolf hybrid, or any mix of the aforementioned breeds (each a “prohibited dog”). This provision does not apply to guide animals used by disabled persons. Birds, fish, hamsters and guinea pigs will be permitted; exotic animals such as reptiles, rodents (other than the hamsters and guinea pigs), ferrets, hedgehogs, or any farm animals are prohibited. Owners are financially and legally responsible for all pets.

Command Religious Program

Nestled among beautiful trees in the family housing area, the Dahlgren Chapel is the focal point for base religious activities. The Command Religious Program delivers religious accommodation, care, and advisement, to strengthen faith, values, and virtues, so that Sailors, Marines, and their families may best serve our country. We currently serve the worship and religious education needs of Roman Catholics and Protestants. The worship schedule is:

Fleet & Family Support Center (FFSC)

The Fleet and Family Support Center is located in Bldg 214, School House Lane behind the NSWC Credit Union. It is a full-service FFSC. Hours are Monday to Friday, 7:30 a.m. to 4 p.m. and Wednesday 10:30 a.m. to 4 pm.

The Fleet and Family Support Center offers the following programs and services for active duty, military family members, and retired military personnel:

Deployment/Readiness – FFSC provides deployment support to address deployment-related issues and to respond to customer and command-identified needs. Programs include Command Consultation & Support, Information and Referral, Individual Augmentee (IA)/Deployment Support, Ombudsman Support, Exceptional Family Member referral information, and Life Skills Education programs.

Crisis Response – FFSC clinical counselors are licensed and credentialed professionals who provide counseling, victim intervention and prevention education related to military quality of life issues. Services include clinical counseling for individuals and couples; Family Advocacy Program (FAP) addressing domestic violence, and child maltreatment treatment and education, and Sexual Assault Victim Intervention (SAVI) prevention and education programs related to sexual assault.

Career Support and Retention - Programs include the Transition Assistance Program (TAP) for service members and their families leaving the military; Family Employment Readiness education programs to assist family members with their job search; the Personal Financial Management education programs promoting financial stability; and the Relocation Assistance Program providing services to outbound and inbound personnel.

The FFSC supports the Command Retired Activities Office staffed by retired military volunteers who provide services to retired military personnel and families to ensure they receive information on retiree issues, veteran's benefits, survivor benefits, and other benefits to which they are entitled.

Pre-Kindergarten:

All students who will be age 4 by September 1 of the school year in which they will begin school will be eligible for Pre-Kindergarten. Pre-K is a half day, educationally and social developmentally based program, which meets from 8:30 - 11:30 a.m. Children may be enrolled in the Dahlgren School Pre-K program, as well as the CDC for daycare purposes; however, parents of children who are enrolled in the CDC for part of the day, and the Dahlgren School program for the other portion of that day, are responsible for ensuring safe transportation for their child to school. The school may be able to assist parents by walking children to the CDC after the Pre-K dismissal.

Kindergarten:

All students who will be age 5 by September 1 of the school year in which they begin will be eligible for Kindergarten. This is a full day program.

Registration Procedures:

All new students will need the school registration form which is available at both the school building and on the school's web-page, located at: http://www.am.dodea.edu/ny_va/dahlgren/. New students will also need a copy of their birth certificate or Registration of Birth Abroad, social security card, immunization records and a current Virginia Physical Form that has been given within one (1) year of the first day of school of the school year for which they are enrolling. They will also need a copy of their sponsor's orders. The Virginia Sports Physical is required only if your child plans on participating in basketball or track and field.

For additional information about the school, the website also provides a copy of the parent handbook and examples of work being accomplished in the classrooms at each grade level.

Observed Holidays:

All federal holidays are observed at Dahlgren School. In addition to these days, school is closed during Thanksgiving holidays, Christmas holidays, and Spring Break. A school calendar, which shows all other closings, as well as information about closures for inclement weather, is included in the parent handbook.

Housing Policy Regarding Enrollment Eligibility:

Dependent children of military members, both foreign and domestic, who are assigned to either Dahlgren or Indian Head and residing in on-base family housing at Dahlgren, are eligible to attend the Dahlgren School.

If a member of the military and his family should move off base after the beginning of the school year, the member's children will be permitted to continue attending Dahlgren School for the remainder of that school year only. Civilian Personnel (GS/WG/AD) who are able to avail themselves of on-base housing are authorized to attend Dahlgren School. They must continue to be employed as a GS/WG/AD employee to remain eligible to attend the school. Children of contractors working for the companies supplying personnel to the base or its facilities, even those who are retired military members are not eligible to attend Dahlgren School, even if the contracted parent is able to obtain housing on the base.

If housing is not immediately available, but is expected within 90 school days from the reporting date of the AF member, the member's children may be permitted to attend the school while residing in one of the areas adjacent to Dahlgren.

Transportation in all instances is the responsibility of the military member.

Dahlgren School has no cafeteria and students are to either go home for lunch or, if both parents work, may participate in the “brown bag” lunch program held at the Chapel Annex. Providing lunch or transportation to appropriate alternative lunch locations is also the responsibility of the parent/guardian. Dahlgren School adheres to and follows the base policies regarding age-appropriate supervision and cannot permit children under the base-set age limits contained therein to transport themselves to and from different areas of the base.

The Installation Commanding Officer will evaluate housing applications to determine whether it is likely that housing will be available within 90 days. While not often the case, if it is decided that housing will not become available in that timeframe, the children will not be eligible to attend Dahlgren School prior to moving on base.

Navy-Marine Corps Relief Society

The Navy-Marine Corps Relief Society is found wherever there are Sailors and Marines. The Dahlgren branch of the District of Columbia Auxiliary is an active, full service office which offers loans, grants, layettes, budget counseling and training courses. The Naval Support Facility South Potomac USO at Dahlgren, VA, is now open to all military members and their families. Residents think of the USO as their “home away from home” and the center provides programs and services aimed at improving the quality of life. This center provides a set of standard USO services which include:

• Local information and orientation through a publication called The Guide to Washington and Baltimore.

• Emergency housing and food

assistance for service members and families with extreme medical,

financial or extenuating

circumstances.

• Information about complimentary and low cost tickets to area concerts, events and cultural activities.

• Information about career fairs for

military spouses and transitioning military.

• Holiday programs which aid in boosting morale of troops and

families during the holidays,

particularly those on duty.

• Support for welcome home and deployment ceremonies.

• Activities and events for families of the deployed including the very

popular United Through Reading

program.

• Lounge services including internet, Wi-Fi, video gaming, cable TV,

reading materials and a large video library.

In addition, the location connects members of the community to a variety of special interest groups that are focused on both families and single service members. Combined with unique and tailored USO special events like Movies on the Lawn and seasonal celebrations, the center is designed to bring the residents of Dahlgren together as a community.

NSWC Federal Credit Union

Credit Union History

NSWC Federal Credit Union was federally chartered in 1961 by ten employees of this base, and has grown into a full-service credit union with total assets of over 269 million dollars and almost 27,000 members. In 2011, the Credit Union will be celebrating 50 years of service to Naval Support Activity South Potomac. We are committed to our heritage, and offer complete financial services to the military and civilian personnel of the U.S. Government who work for or are permanently stationed on base or one of its tenant commands.

It is also important to note that our members' savings are federally insured to at least $250,000 and backed by the full faith and credit of the United States Government National Credit Union Administration, a U. Second Tour Thrift Boutique

The Second Tour Thrift Store is located at 722 Sampson Road and is operated by the Dahlgren Officers' Spouses' Club. Business hours are Thursdays from 9:30 a.m. - Noon. The Second Tour accepts donations of gently used clothing and household goods. Free military uniforms are available 24 hours a day. All proceeds raised at the Second Tour are returned to the Dahlgren community through scholarships and charitable donations.
